This is the letterhead: your name and address go at the top of the request, and the association's name goes underneath. Your right to inspect records comes from living there, so the address matters.

Tick only what you actually need. A focused request is quicker to answer, cheaper if copying charges apply, and much easier for you to hold them to. You can always ask for more later.

Inspecting in person is usually free and you may photograph what you need. Asking for copies is easier but the association may charge for them. Tone changes the wording only — either version asks for exactly the same thing.
